  • Complete Forms in CampMinder
    • Tax Forms (W-4, I-9)
    • Health Forms (Physician’s Exam, Immunizations, Health History, Insurance Card)
    • Work Permit Form (if your 18th birthday is on or after June 10, 2025)
    • Travel Form (For US-based staff)
    • Staff Additional Form
  • Upload current photo in CampMinder (to be used on your staff ID badge)
  • Book flights (if you will be flying to Chicago to take the bus to camp)
