- Tax Forms (W-4, I-9)
- Health Forms (Physician’s Exam, Immunizations, Health History, Insurance Card)
- Work Permit Form (if your 18th birthday is on or after June 8, 2026)
- Travel Form (For US-based staff)
- Staff Additional Form
Upload current photo in CampMinder (to be used on your staff ID badge)
Book flights (if you will be flying to Chicago to take the bus to camp)
